Start a form request

Create a new fill request from Forms Center.

Open Forms Center and find the form you need. Check its name and edition before starting.
Choose New Request. Select the client or case and complete the steps shown for fields and data sources.
Review the request before creating it, then open the case's Forms tab to track its progress.
